Every counter bill is an ordinary GST invoice, so your cash book, customer dues and GST reports are up to date as soon as you press Pay. It is in the menu as POS Billing, on every plan.

Yes. Open Return items, find the bill, and choose what came back. The refund is given in cash or taken off the customer’s dues, and it is recorded as a credit note. In an exchange, what came back pays towards the new items.
